Rev. 5/20/j PROPOSAL TO ESTABLISH A NEW MINOR 1. Overview A. Description Provide a brief description of the proposed minor. The minor in community health is designed for students who want to create healthy, sustainable communities with an emphasis on working with specific populations. Community health is a specialized field within public health and concentrates on the actions and conditions needed for the health and well-being of defined groups of people. Graduates work with teams in diverse settings to formulate research-based strategies that assist communities to make healthy and sustainable lifestyle choices. B. Why should this be a minor rather than a concentration? The minor in community health will be open to students in all majors and will provide the opportunity for students to add this knowledge to their primary degree program. Examples of students who may have an interest are nursing, social work, education and nutrition. C. Affiliation If the minor is affiliated with a degree program, include a brief statement of how it will complement the program. If it is not affiliated with a degree program, incorporate a statement as to how it will provide an opportunity for a student to gain knowledge or skills not already available at ASU. The College of Nursing and Health Innovation will also offer a BS in Community Health. Students who want to develop a full range of skills and knowledge will be encouraged to select the major. The minor will serve the needs of students in other majors who want to expand their knowledge base and skill set in the area of community health. D. Demand Explain the need for the new minor (e.g., market, student demand, interdisciplinary considerations). With the implementation of the Affordable Care Act, the demand for community health workers will increase. Graduates of this program will provide health education, health promotion, care coordination, health care navigation and advocacy services for individuals, families and communities. The demand for these services is increasing as more health care systems are providing resources to help individuals remain in the community and avoid hospitalization E. Projected Enrollment What are enrollment projections for the first three years? Number of Students (Headcount) 1 st Year 2 nd Year (Yr. 1 continuing + new entering) 3 rd Year (Yr. 1 & 2 continuing + new entering) Support and Impact A. Faculty governance Provide a supporting letter from the chair of the academic unit verifying that the proposed minor has received faculty approval through appropriate governance procedures in the unit and that the unit has the resources to support the minor as presented in the proposal, without impacting core program resources. The College of Nursing and Health Innovation (CONHI) Academic Assembly and UG Curriculum Committees have voted to approve this minor. Students in the minor will enroll in courses currently in the major degree program. No additional resources will be required to offer this minor. B. Other related programs Identify other related ASU programs and outline how the new minor will complement these existing ASU programs. (If applicable, statements of support from potentially-affected academic unit administrators need to be included with this proposal submission.) None C. Letter(s) of support Provide a supporting letter from each college/school dean from which individual courses are taken. None. All courses included in the minor are from CONHI. 3. Additional Enrollment Requirements If applicable list any additional enrollment requirements students must complete. A student wishing to pursue a specific minor should consult an academic advisor in the unit offering that minor to ensure that an appropriate set of courses is taken. The student should also consult an academic advisor in the college or department of his or her major to make sure the college or department of the major allows the recognition of the minor. Courses taken as part of a minor may not count toward both the major degree and the minor. Please contact the department for more information. 6.
